Life Transition Services offers comprehensive support to a wide array of needs that promotes the life each individual desires.

 

Residential

At Life Transition Services we recognize that where and with whom you live largely influences the quality of everyone’s life. For that reason, we will carefully assess the best match for each individual.

Enhanced Family Care – Life Transition Services cultivates the best matched home for your needs and provides for on-going support to ensure a quality of life for each individual we serve.

Interdependent Living – Certain individuals may be best suited for a more independent living arrangement. Life Transition Services has apartment options for such individuals in an adjacent building to our main facility.


 School-based Support

Our Life Coaches assist select students to remain within the school setting for classroom instruction, related services and other curriculum according to each student’s academic schedule.  Life Transition Services offers this model of support for part or whole day programming.


Independent Living

Students learn how to take on the actual life responsibilities in the manner in which they will be taking them on after graduation.

Functional Learning

Students put into practice the academics they have learned in the classroom in real life settings.  

Educational Services

Life Transition Services offers modified regular programming to area schools for select students.  Life Transition Services, along with the IEP team, will design either a school-based or community-based model of support.

Community Participation

Life Transition Services’ Life Coaches enthusiastically assist others to have meaningful connections and contributions in their communities. Our Life Coaches help enrich lives and create opportunities based upon the individual’s abilities and interests within the following (3) activity categories:

Independent Living – Individuals further develop skills necessary to gaining independence and competencies in actual life responsibilities.

Functional Learning – Individuals put into practice the academics they learned in the classroom in real life settings.

Recreation/Leisure – Our area invites a variety of both indoor & outdoor recreational opportunities from 3 fitness centers, swimming, horseback riding, water-skiing, kayaking and skiing/snowboarding. Participation in the local Special Olympics is also an option.


Supported Employment

Employment is our passion! We believe that the strongest sense of accomplishment, achievement and contribution is attained through one’s work. Therefore, employment development has been the foundation and origin of our work in the Connecticut River Valley. The majority of the adult clients and graduating students that we have worked with have employer paid jobs of their own! Each job seeker benefits from the following employment curriculum:

Vocational Assessment – Each individual is screened using an interest inventory to develop a career profile.

Pre-Employment Skill Development – All job seekers will learn skills necessary for resume prep, interviewing skills, completing applications and employment portfolio.

Job Shadowing – Based on career profile, each individual visits a minimum of 3 job sites within each area of interest.

Work Experiences –Prior to job placement, a work experience is helpful to ensure job match.

Competitive Hire – All job seekers at Life Transition Services are assisted to explore meaningful contributions either through volunteer commitments or a paying job of their own!



Community-based Transition to Adulthood

Life Transition Services also offers a vital link to area schools and select students in developing IEP Transition to Adulthood goals.


In-Home Support Services

Life Transition Services is a vendor of In Home Support Services to families who have an IHS Waiver. We offer the following support services to such families and their student:

  • Respite – The value of a much needed break offers both the student and the family an opportunity to be refreshed for the journey ahead. Life Transition Services can take the stress of finding well 
  • Community-Based Support - Life Transition Services Life Coaches enthusiastically assist others to have meaningful connections and contributions in their home community after school and on weekends designed around the family’s needs.

 Career Development  

Over 95% of the students that we have worked with have had employer paid jobs of their own upon graduation!
