In-Home Support Services
Life Transition Services is a vendor of In Home Support Services to families who have an IHS Waiver. We offer the following support services to such families and their student:
Respite – The value of a much needed break offers both the student and the family an opportunity to be refreshed for the journey ahead. Life Transition Services can take the stress of finding well
Community-Based Support - Life Transition Services Life Coaches enthusiastically assist others to have meaningful connections and contributions in their home community after school and on weekends designed around the family’s needs.
