In-Home Support Services
Life Transition Services is a vendor of In Home Support Services to families who have an IHS Waiver.
Community Participation
Life Transition Services’ Life Coaches enthusiastically assist others to have meaningful connections and contributions in their communities.
Educational Services
Life Transition Services offers modified regular programming to area schools for select students. Life Transition Services, along with the IEP team, will design either a school-based or community-based model of support.
